Oxytocin
Oxytocin (cyclic nonapeptide)
Social bonding and intimacy peptide
About
Oxytocin is a 9-amino-acid cyclic peptide hormone produced in the hypothalamus and released by the posterior pituitary. Approved by the FDA in IV/IM form for obstetric use only (labor induction, postpartum hemorrhage, milk let-down). Off-label research interest centers on intranasal use for social cognition, bonding, anxiety, and sexual function, though large trials have produced mostly null results.
Mechanism
Binds the oxytocin receptor (a Gq-coupled GPCR) in the CNS and peripheral tissues, modulating amygdala reactivity, prosocial behavior, smooth-muscle contraction, and reward circuitry.
Dosage
When to take: Dose 30-60 minutes before the targeted social context. Effects on behavioral endpoints typically measured 30-90 minutes post-administration.
Reconstitution
Vial size (mg): 2 mg (~1,000 IU) · BAC water (mL): 2 mL · Concentration: ~500 IU/mL (1 IU ≈ 2 mcg)
For intranasal use, transfer reconstituted solution to a metered nasal spray bottle; 10 units on a U-100 syringe ≈ 50 IU.
Benefits
- Possible enhancement of social attention and trust (mixed evidence)
- Reported increase in feelings of bonding and intimacy
- Approved efficacy for labor induction and postpartum hemorrhage
- Anxiolytic effects reported in some small studies
Side effects
- Nasal irritation and sneezing (intranasal)
- Transient headache
- Irritability or emotional blunting at higher doses
- Hyponatremia and water intoxication (high IV doses)
- Uterine hyperstimulation (obstetric IV use)

Research
- Intranasal Oxytocin in Children and Adolescents with Autism Spectrum Disorder
New England Journal of Medicine · 2021
SOARS-B, the largest RCT (n=290), found no significant difference between intranasal oxytocin (up to 48 IU/day for 24 weeks) and placebo on social withdrawal or any secondary social endpoint.
- Oxytocin enhances amygdala-dependent, socially reinforced learning
Journal of Neuroscience · 2008
Single-dose intranasal oxytocin modulated amygdala responses and trust behavior in healthy adults, supporting a CNS-relevant pharmacology despite limited bioavailability.
- Intranasal oxytocin versus placebo for hypoactive sexual desire disorder in women
Fertility and Sterility · 2017
Small RCTs of intranasal oxytocin for sexual desire have shown modest or null effects on arousal and orgasm endpoints versus placebo.
